The Fieldhouse Sportscenter realizes that not all individual players have a team to play on.  We will attempt to locate an appropriate team/coach for any player that requests our assistance.  Simply complete the following information sheet and submit it to us and we will do our best to find a team that fits your athlete.  Filling out this form does not guarantee a spot on a team, however we will do our best to get a team for your player to play on.
